When Customfield value is selected exception can there be a note field that is required that pops up

Hello ,

I have multiple Customfields with values completed , N/A , Exception . My requirement is when they select exception  can there be a note field that is required that pops up .

Does it seems possible can anyone help me out on this with required suggestions .
