Some week ago, when I moved one previous Service Request over to a Change Request instead, these three fields started randomly popping up all over our tickets:
How do I get rid of them? And why did they appear in the first place?
It happens on both Service Requests and Tasks, to my knowledge. Not sure if other types are affected as well.

These fields are added automatically when you enable Change Management in your Jira Service Management. More information
I think you can either disable the "Change Management" feature from your project:
Or update the related screens to remove the fields.
