Unable to create an issue using a link to a form

Have created a form and followed these instructions to create a shareable link. However, upon opening the link and selection the appropriate project, I get the message "This service project doesn’t have any associated forms" which I don't understand since the form is clearly saved within the selected project.

After enabling the create a shareable link and selecting the request type, you need to click on the save changes at the bottom. Then copy the link and test.
