Stuck in an endless loop when trying to connect Jira to Slack

I am trying to connect a Slack channel to a Jira project but am getting stuck in an endless loop.  I tried the recommended fixes, such as:

• Clearing cache and cookies from browser
• Using incognito mode
• Using different browsers (Chrome & Edge)
• Trying from my phone (iPhone)
• Rebooting

Nothing changes the outcome which I will walk thru with screenshots below:

Step 1 - Try to connect jira to the slack channel, and I get the following

Step 2 - Click "Log In", then "Install Jira Cloud", then in Slack it says I'm connected

Step 3 - Back in Slack, type /Jira manage, and I get this same message again.  If i click Log in, it loops me back to step 1

My IT team is also stumped so that's why I'm here

Your screenshots don't seem to have been posted, can you post them again?

Could it be related to slack instance permissions?

Odd.  I just logged in again, and I see the screenshots on my end.  Added them again here:
