We have a JIRA Service Desk project with Request Forms based on issue type and request type. Once a ticket is created, we can add a form to it, but this form needs to be completed by our external customer. We want to add the form to the create ticket page so external cxustomers will be prompted to fill it out at in-take. We cannot figure out how to have the "Add Forms" button to the Create tickets screen for JIRA Service Desk. Can someone help?
Hi @Chasmin Lewis , great question! When editing the request type from your project settings, you should see an option at the bottom of the request type editor to attach a form. This will add the form into the Request form view that the customer sees in the portal.

You can add the forms to the request type in the project settings after clicking on the request type you need to add the form. At the bottom of the screen you will have option to attach the form. For reference i have added the screenshot
