Proforma Form API URL is not accessible showing 404

Hi, I tried to use the Proforma Form API using the below URL but it is showing 404.

Please someone help me to get the actual URL and how I can get the form id 

below url shows 404

/rest/proforma/1/issue/{issuekey}/form/{formid}/answers

Hi @jiffy_sandbox,

Welcome to Atlassian Community. Do you mean ProForma: Forms & Checklist for Jira ? It is available only for Server or Data center version, not for Cloud (as indicated in this post in tags). 

Error 404 mean "Not Found". It looks like the ID form doesn't exist.
