Linked Slack Chat with Jira Service Management

We linked Slack Chat with JSM but the JSM request types are uploaded form for users to fill out. On the Slack Chat channel when a ticket is raised, the form does not populate. Is there a way to get the form on there?

When you setup the integration between Slack/JSM, there is a step where you configure the specific JSM request type to facilitate the issue creation in JSM.

Thank you for this. I already have created the chat with slack. All of the feature works except the upload form where the fields are located are not showing up. Do you know if there is a way to get the form linked to the request on slack?
