Jira automation error and confluence form Intergration

Hello I have a few issues 

I did the automation for a meeting request off portal form

• condition > when issue type = meeting 
• then create a subtask in the same project >issue type subtask> from the trigger issue 

(problem is when it triggers the subtask fails )

Error creating issue

• zone is required. (customfield_10059)

Error creating issue

• Issues must be assigned. (assignee), Zone is required. (customfield_10059)

How do I get the automation to 

• meeting request> create subtask x5> assign to members in the meeting to review the notes 
• the task must be date locked to 1 week before the actual  meeting date from the meeting request
• ------------------------------------------------------------------------------------
• action items from the jira form embedded into the confluence page along with the meeting notes
