We want to manage change requests with many managerial approvals in IT. On the other hand, we will need Jira Cloud Standard licences for other projects. Approvals can be manage in Jira Premium licence too.
Which product I should choose? Should we buy Jira Cloud Premium Licences or Standard JSM for IT + Jira Cloud Standard? Do approvers need licences for JSM?

The decision should be based on your specific requirements for project management, the approval process, and the complexity of your other workflows and processes. An "always fits" statement is hard to make.
Jira Premium also offers Plans, which is a great tool for managing projects. Additionally, sandboxes are useful in daily business operations.
Do approvers need licenses for JSM?
Typically, they need a license to participate in the approval process and to see full ticket information. Tickets in the customer portal are not fully visible, so it can be difficult to make a decision without complete information. I would recommend obtaining licenses for approvers.

Approvals can be done in both JSM Standard and JSM Premium. You do not need to get Premium just to do approvals.
Approvals can be granted by JSM Customers, who do not need to have a license. But members of your team who are doing the actual work of updating and transitioning tickets/requests, will need to have an Agent license.
