Is there any attachment field in the Jira form that requester can attach a file to a form request?

I need to have a feature in my Jira form that the requester can attach a field to a form. 

How can I add this field to my form?

If you are referring to the FORMs element of JSM, then it is not supported to include the default "Attachment" field in a FORM configuration.  However, in your Request Type screen configuration, you can include the Attachment field as a part of your Request Type screen setup and at the same time to include your FORM.
