Is there a way to have a form appear when a user populates a field?

I have a client who has asked that a form be popped up after a user checks one or more options in a check box field.  This form is essentially a legal disclaimer about the items in the field, and they will need to essentially OK and sign the form. 

This needs to be done if users enter a ticket in Jira directly, or via the customer portal.

something like dynamic-forms might be able to help you. you can create conditional forms for the jira issue & for the portal.
