Is it possible to add an actual attachment to a form?

I would like to add an actual attachment to my form, for people to click and download when creating a ticket.  Is this possible?

Unfortunately based on my knowledge, it is not possible.  However, you may may want to add an URL link to the document in your FORM's design, so your customer can access the URL to obtain it.

I have not personally tested this out, but it should work if the URL is accessible for your JSM customers.
