I can't seem to create issues with a form

I can't figure out where I'm going wrong. I've added forms to the workflows and request types, but when I try to make a link for create issue, I select my project, then get this error "This service project doesn’t have any associated forms." I dont understand. I made the form in the project, how do I associate it??

Could you please clarify "I select my project"? Could you add screenshots?

When I'm in the form, I see that only the request type is required to choose:

It lets me pick my project there, but then I click on the link and get this

@Ellie Kerns I couldn't find such a view. Could you please send a bigger screenshot to show the context? Where is located? It looks like a global view which I can't find on my Jira instance.
