How to format the amount field and the date field in the Service Management Form

How can I set up the amount field to display a thousand separator and two decimal places, and the date field to display as DD/MM/YYYY in the Service Management Form?

Setting you own separator is not possible, there is an open feature request for this; JRACLOUD-75109
