How to edit Issue pages with radial buttons

My company is currently using JIRA Service Desk, and there are customizations with radial buttons for the selection of Employee, Contractor, Third Party. I'd attach a screenshot, but my laptop is thwarting me.

Now that we are working toward our migration to JIRA Service Management in the cloud, we'd like to duplicate some aspects of our previous Help Center Issue entry with the same radial buttons. I can see date selectors and such, but no radial buttons to use.

    • Create custom field

Keep in mind that once you create that field, you have to add it to the appropriate screens you want.

In your portal you create your request types. 

Within  your request types,  you configure your fields. This is were you place your field.
