You should be able to use Automation to do this.
It might look something like this...
- Create the first issue manually, with a unique identifier (eg. a label)
- Create an Automation rule which runs once a day and looks for all issues with that label
- Check there's no open issues
- Check the last closed issue's resolution date (if all are closed)
- If it was 3 months ago, create a new issue
