How to bring the group name (visible in portal) into the request ?

I have created customized groups. One for each of my applications, then I have associated request types. Now, I would like to see the group name (my application name) into my request form.

Should I create a new field ? Please help with the steps. I am a beginner without computer science background.

Welcome to the community !!

Currently JIRA Service Management request form supports all custom fields except Group pickers, Project pickers and Version pickers.

Workaround: 
You need to create a custom field with type select list (Single choice or multi-choice) and configure its options with your group names.

Please note: The field values must be updated every time when you create a new group.

When i create a new project from a template, after 'Project Overview' there is a drop-down for 'Group Name'. Where can I edit/add/remove items from this list? I want to change the groups available to me in that drop-down but can't find it anywhere in settings. lcps go

Welcome to the community.  Can you provide a bit more information on "customized groups - one for each of your application"?  What do you mean by seeing the group name in your request form?  How are those groups associated to your request from currently?

Once we received your clarifications, we can then advise further.
