How do i get my custom form to show in widget

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
I am trying to setup a widget to drop in my company site, but the widget is not reflecting the related form. It only shows fields for Summary and Email contact.
How do we get our custom form to show in widget? This issue was raised in 2024 but there is no official response
