Full field set for ITSM best practice

Hi there.

I'm creating a new form in the Jira Service Desk.

It is an ' Ask a Question' - when I go to save the form a pop up appears that there are missing fields and I need to make the issue type compatible with the form.

I'm new to this and I am unsure what it is I need to add.
