We use the jira board for contract requests and would like to upload contract templates in fillable pdf format for requesters to select when they submit a request. Is that possible?

Community moderators have prevented the ability to post new answers.
If you want to collect information when people submit a request, using a form with Jira Service Management or a more basic one with a Jira Work Management project would be the best solutions to leverage the functionality of Jira.
Hi @Rebecca McMahon have you looked at some of the apps on the Marketplace that allow you customize pdf exports? I think you will need to look beyond the native functionality of Jira.
