FR: Can't **keep Form in Request type** when changes are made to Form

Hello,

it appears that every time I make a change to a Form, it gets automatically deleted from the requests type(s) it's embedded in.

This is really annoying and doesn't make sense, because I'm only improving the form so that it looks better/fulfills our needs better in the request that it's embebbed in....... since I'm finishing up the set up for our Support Portal now (upcoming launch), I'm constantly making changes to the forms and it's very time consuming and annoying to have to go back into every request type config to add the forms again.

I've been able to repro with several forms and different request types. Same result.

Please fix this bug soon...
