I'm using Jira Service Management for support departments in my company. ( Admin, Design, HR )
When I created projects for each department, I used templates provided by Jira.
So, projects have been configured for request and issue type. I also opened the portal and created requests successfully.
The question is: can I edit the Issue type form provided by Jira?
For example, In [Design Service] template has [Request a design access] Issue type.
I need to add some more properties to the [What kind of design asset is required?*(required)] select box.

You can edit the forms (and other settings) of projects after you create them. It is just worth mentioning that any changes you make apply to the project itself, not the template you originally created it from. That template is not editable for you as a user. That means that if you would create another project from the original template again, it would still start with the same settings as you see now.
What you can do (if your JSM projects are company managed - which they probably are), is edit screens, workflows, ... from one project and link the configuration schemes of that projects to your other projects. That way, they get a shared base configuration, so changes are applied across all your projects at once.

Here is project setting of my project. I created it from design template.
