Carregamento automático do solicitante

O novo departamento tem um formulário chamado Risco Operacional, que utilizei um template. Porém, não consigo adicionar nesse formulário a captura automática do solicitante, para que seja preciso preencher.

Como consigo incluir esse campo?

Welcome to the community.  Have you tried to create the FORM using JSM Form component app?.  When users fill out the FORM configured/setup using this out of the box app of JSM, he/she will automatically be the issue reporter.  You can also set it up to be used for issue creation for your JSM project.

Unless your existing Form was already built using the Forms app, then please advise how users use the Form to create JSM issues?  Are the users who fill out form himself/herself, or it is used in your custom process to create the issue while the form is treated as an attachment.  This means that the issue creator is not the actual person who filled out the form.
