I'm not looking to the option on using the form. Rather is there a way to make the custom field hidden or visible depending on the field selected.
Example:
If I choose category I would only like to populate a notes .

@Kristine Mamangon When you create a request type in Jira Service Management there is not a way to make those fields conditional except for cascading lists https://support.atlassian.com/jira-service-management-cloud/docs/add-a-conditional-field-to-a-service-project/
The forms product allows you to make things conditional but that will not be on the request. Form fields can be mapped to custom fields.
