Can't add a Date&Time field to an issue type request form

Given:

I need to have a Date and time (one field) field added to the request type and a form for this request type, in request type where I am supposed to add fields there is no option to add a field for both Date &Time. 

I have created a condition field Date&time one and connected to screens, but I don't know what to do next. I still don't see any option to add this field.

Would appreciate your advice.

I believe this article is what you need. Any chance to apply those steps in the article before?

I dont' have an option to add time in the Request type part where you are supposed to set fields and ability to create them for the request.
