Can a form's questions and answers be (cleanly) shown on an issue without having to open the form?

We've created a form with custom fields that automatically submits an issue, and then we break down the form into sub-tasks. I'm aware that the issue layout would be able to display the name of the custom fields and their answers, but the problem is three-fold.

1: We need to be able to break down sections of the form into sub-tasks, so we need to show only relevant information to our desk engineers. The form contains a lot of information that can lead to confusion.

2: The custom fields' names are used to identify them when linking Jira Fields, and therefore are not adequately descriptive enough for our desk engineers to intuitively know what the question refers to. If we used the question field label (as it's referred to in the form builder) as the custom field name, it would be confusing to see 3 different fields referred to as "Name" or "Launch Date."

3: These fields cannot be hidden if left empty. Not only would this be a pain to navigate through over 300 custom fields, we would not know if there is missing info without opening the form and scrolling through to the relevant sections.

I want to know if there's a way to automatically show the questions and answers filled out in the form somewhere in the description field or where the issue fields are normally displayed.

 

My current workaround is to clone the issue, hide custom fields in a new issue layout, type out the form questions in the description then typing each respective smart value beneath them in automation. This gets difficult to maintain when we want to change the form's questions, because I'd have to update them manually.

Now I have to consider data connections too. I couldn't find any smart values associated to them, so now I'm not sure how to display them. Any advice is appreciated, thank you!
