Can I format the request form that the requestor fills out?

I am trying to edit the form that the requestor fills out so that I can add titles and divide the form by sections.  Is this possible?  I have a request that the marketing managers fill out to request email sends and want to be able to add titles to the form so it makes sense to them. 

I created more custom fields but would like to add a title.

Hi @Marcela Rojas , if you mean the new Forms feature go to project settings > Forms and locate the form in question an click on the ellipses on the right and select edit. If you mean the original form that is part of the Request types then go to project settings > Request types and edit the request type in question.

I am using an older version of Jira.  Here is a screen shoot of part of the request form that the marketing managers fill out. Not sure if I can add titles to it.

If you have admin access then proceed to project settings and request types. You can absolutely modify the forms present it to the customer. If you are not a project admin then try to reach out to someone to assist with this.

Yes, I have admin rights but when I go to request types the only option I see is adding or removing fields. Wondering I can add just titles, not fields. My goal is to have titles to divide the form in sections so the customer has a better experience.

I was thinking, for titles I need to create a new custom field just to display and use for a title maybe?
