is there any way to have the approver write a comment, especially when declining a request?

For non-agents (i.e from within the customer portal), I don't believe this is possible at the moment, as it's not possible to have a screen as part of the approval - nor is it possible to ensure the validators activate when approving from the portal.
There are feature requests for this, check out JSDCLOUD-6989 and JSDCLOUD-6988

An alternative might be to remind approvers to comment on the ticket using Automation? Not a direct replacement, but an option?

You could achieve this by adding the Field Value Changed Validator on step 8 of the following KB article.

Follow the article and after step 7 add "Field has been modified Validator" to achieve a mandatory comment without a 3rd party app.
Otherwise you require a 3rd party app.

I already tried that. Doesn't work in cloud. When the approver hits "Decline" button the request/issue is transferred to Declined-Status.

I just tried it in a project in my cloud and it works, agent perspective
But it's on an approval step, if this is done from a non agent perspective, screens don't apply.
