All forms in One Page

Hi Team,

We need something like Quick links in Jira.

We work in a Jira Service Management Project and created some request types as forms for customers to access. Now we have problem like, we create multiple forms for every project. So if there is anything like a page where it can show all those forms for the customers and they can submit which one they want.  Just like hosting all those forms in a single page with Project Name or the Form name to view the form. 

Since we have multiple forms to be created for HR, Procurement, IT, Legal, Marketing, Finance, Sales, etc..,
So this would have a larger forms and I also posted a previous question like form is showing as slide type and we wanted it like a excel-form, so is there any way we can add excel into form with formula like that. I tried that, but no response for the question

Please give us the solutions for both requirements.
