Affected services dynamically adding specific form

Hi, I'm wondering if it's possible to have different forms only show based on an affected service. I've seen with 1 form, you can conditionally show based on input however it is not supported with the multi-select field "affected services".
I'm currently working with 1 form, and a custom checklist field "services" but having issues linking this to Azure DevOps via integration tools. I believe using the multi-select "affected services" I will be able to integrate better.
