Adding an acknowledgement on portal

I would like to add an acknowledgement on my portal that requires the customer to say okay or click on checkbox. The acknowledgement is about personal data and will need a small paragraph to explain our process with this data. Looking for suggestions on how to set this up. I tried using a checkbox custom field but my text is too long for that.

You can add a checkbox customfield (Ex name; GDPR) and add paragraph as a description to this field.
