Add form to widget for website

Hi,

I noticed if I have a custom form embedded into a Request Type it shows fine on the desktop browser. However once you configure a widget the widget only shows basic fields but not form.

 

How can I show custom form in the widget while creating a new ticket of a specific Request Type?

bumping this. the support widget is basic and i need a solution to ask relevant questions to cut down on back & forth with client.
