In the output of the Managed Accounts report, when the Last Active Date field is blank, what does that mean? Does it mean the user has a license, however has never accessed the product, even to view?
Not necessarily! Blank Last Active Date just means no activity, it doesn't automatically mean they're taking up a paid license, since managed accounts can exist without product seats.
