I have tried a couple different extensions and/or power-up and for some reason, none of them that set backgrounds colors (usually based on 1st label assigned) for cards sem to be working. Instead the card background remains white and the thin label sliver of color is displayed. I have deactivated and uninstalled all other Trello extensions and power-ups. Selecting a card cover color and choosing the full card option DOES work.
Any ideas?

I've had the same thing along with many others it seems. I've been using these extensions for a long time before to get a better "colorful" view of the boards but now none of the existing extensions to set the background color of a card depending on the label (color) works. I've tried them all. Altlassian/Trello did an update in September last year (I think it was) and all of these extensions stopped working.
If you look at some of the extensions and their support sections in the Web Store, many users seems to have the same issue.
I've been waiting for any of the extensions to be updated so I can use the label=background color as I've always been using in the past.
As you say selecting a card cover/color and choosing the full card option does work but it's not as easy as selecting label #1 and the card background automatically sets the label color.
I'm hoping for an update of any of the available extensions or if Atlassian/Trello could implement an option to - if label #1 is assigned then apply label #1 color as full background color for the card or something...
I guess it's good to know I'm not alone. It's unfortunate that it's even possible for tis to happen. I thought that being part of Atlassian might have tightened things up a it, but I guess not that tight... yet.
Although this is the most correct answer at this very moment, I'm going to leave this unresolved in case a solution *does* surface at some point in our lifetime.

It sounds like you may have the wrong browser settings enabled. Try disabling any browser extensions or plugins that may be interfering with the card background color option. If that doesn't work, try using a different browser to see if the card background color option DOES work.

It's not a browser setting - I've disabled all Powerups/extensions (except this one obviously) and tried on different computers... all with the same results. It sounds like others are experiencing the same thing.
