What causes random double posting in comments?

We have begun to notice random double posting in the comments.  Has anyone seen this before and can share their observations and hopeful solution?

Hi, Pete!

I see we had a similar discussion a couple months back and I also worked a case where it was caused by the plugin failing to initialize mentioned on this other question:

A part from that, let us know whether these comments are being made directly on the issues or coming through e-mail via mail handler.

If the issue is reproducible, see if the user is clicking the button multiple times - normally Jira wouldn't create a duplicate comment by doing that but other factors could play a role.

Is this issue resolved yet? Because we are experiencing the same problem after upgrade to V9.12.8 datacenter.

@Ha Tran hello! I see there's a bug that has been fixed but not on your version:

When you get to 9.12.13. If that's not quite the same you may want to submit a ticket to review it.
