I am trying to build a structure in the following way
Initiative
Epic
Stories
I've linked the Epic to the Initiative (as parent) and I can see that the stories are included in the epics. I've tried to ''Extend - Link issues'' but it does not work. Any idea why?

Their might be the following reasons for you not able to extend link issues:
Permissions on linking issues.
Restrictions on the issue type or workflows.
I would suggest to have a word with the your in-house Jira admin.

When Adding a Generator to a structure, on-prem, you will want to select the name of the structure from the top row before selecting "Add".
In your case, it would be the "DSRHC Struc_Test" text just above the "Insert Issues:..." generator.
