I need to clone a specific ticket multiple times when it's submitted, including the form that was filled out.
In automation, I see selections for Copy and Attach forms but neither of these seem to bring in the form. I have seen previous posts where cloning the form was not possible but is this still the case? If the forms can't be cloned as well, what is the purpose of having the Copy/Attach form options?

As an alternative to Automation (that can be complex sometimes), I would suggest trying our app Elements Copy & Sync to clone issues with their attached forms.
The configuration is really simple, you can check the guide here.
The app is for free during 30 days (and it stays free under 10 users).

Cloning forms directly when cloning an issue through automation is indeed a challenge in Jira at the moment. However, our app Deep Clone for Jira, can be an excellent solution to address this limitation.
Deep Clone for Jira enables you to clone issues along with many associated elements, including attached forms. You can read more on that here.

You are correct: cloning forms directly is still not supported for automation rules.
Here is the suggestion to add that, including a workaround where the rule calls the REST API to copy the form: https://jira.atlassian.com/browse/JSDCLOUD-10939
