Indexing Failure when transitioning issues to subsequent status in workflow

Hi,
I encountered this problem of indexing failure when transitioning issues along the workflow (No specific status, randomly occurs but with a 100% chance of occurring).
I went into my H2 database and deleted the issues and re-indexed, but the problem persisted. Now for some reason I am unable to re-index without facing an error.
Need advice on this.
