I need to find an app for templates, so that we can collect data from customers in a form. For example if they select Template "Games" a set of fields is played like this.
We had "Templates for Jira issues" plugin for data center but it is not supported by Cloud.
The main functionality, that app should have is to a it in Customer portal, making visible for clients.

Could you please confirm that the DC version of the app you use is Issue Templates for Jira provided by Deviniti? If not, please note that our DC verison of the app is very similar and we also have an alternative version for Jira Cloud. When it comes to variables on the Customer view is a bit limited for now, however, we're planning to implement this ability and hopefully, it will be technically possible soon. I

The customer portal in JSM out of the box already provides you with request types. These are a type of forms by themselves, that allow you to prompt your customers to fill out detailed information like in your screenshot. This support article is a good starting point to read more about them.
Just by themselves, request types lack a bit of flexibility, which can be resolved by adding forms to them - also an out of the box feature of JSM. This support article should get you started on what they are.
Use the links provided in both articles to catch up on how to create request types and forms, as well as dig into the more advanced things you can do with them.

Thank you for your response. I read articles and tried solution, but still my request is not completed.
Please advise the following. I need to create up to 20 Forms, and customers who rais request should select 1 form out of 20 and fill it. Now when I add all those forms will be displayed on create screen.
Is it possible to select from Dropdown Form name?
And another question - can I add Forms information directly to description?
