How can i create a deployment ticket in JIRA, It should also contain set of questions.

We need a separate dashboard for this,  deployment ticket where we can answer these questions just tick mark or right comments and then, below there will be submit button where a ticket will be created and then it can go for approval to CAB and other work flows.
