Other than going through each project manually, there is no way for me to identify the missing items. Why does this happen / how do I prevent?

Can you share with us the filter?
How many results is it getting, total? If it is getting 1000 issues total, that is the maximum that can be retrieved by a filter.
If you can provide us with the filter, can you also provide us with a screen image of an issue that you think should be included but is not? Then we can compare that issue to the filter.
It could also be that the filter you created does not actually get what you intended. Can you describe to us what you want to retrieve?

The filter you are showing us is the Basic view. Notice the +9 in the Status pull-down? Subsequently we can't see all the details of the filter.
Can you please click on the JQL button, and then paste a copy of the full JQL statement?
