Does Jira Service Management have the functionality to link forms together?

I would like to set up a form whereby clients click one out of three options. After they click that option, it takes them to a second layer of questions which they fill out. Is this possible?

So what I have sort of found is that you can use radio buttons and conditionally sections to make forms appear if the user selects the appropriate button. 

However, I am still trying to figure out how things get triaged appropriately. I.e I have 3 options at the begging of my form. I want it so that if they select option, it takes them down path A, if they select option B then path B, etc. 

Does anyone know how to create a form like that?

I am looking for the same fonction too
