How do I create filter to know how many times user story been Returned In Development

The situation is:

1. Development moved user story to QA for testing

2. QA team tested and the user story failed

3. User story Returned in Development

4. Dev. fix it and moved to QA again

5. etc.

The questions is: How do I create a filter to know how many times user story been Returned In Development and how many times this user story been tested by QA
