We'd like to to insert arbitrary headers (static html) between fields and custom fields in a single service request.

This might take the form of

Hi Aaron,

You can use the JIRA Toolkit Plugin and the directions in How To Add a Message or Field on JIRA Create Issue Screen.  Instead of creating the Tab, just add the fields where you need them to be on the screen and use html for url links.
