I've created a Validators in my workflow as follows.
Custom Field 1 and Custom Field 2 is required before approving the request but it's not working.
We decided that Custom Field 1 & Custom Field 2 would not show on the actual form but available on the Agent view Description field.
We wanted our HR Team to assign a value for Custom Field 1 & 2 before they can approve it.
If you setup the validator, then you will need a pop-up screen (associated to the WF) with those two fields when the transition is triggered. From there, the screen will not allow the transition to complete when those fields are empty.
Otherwise, your validator setup is not going to work.

Did you added the same fields that you setup for your validator configuration in the screen?
The screen should contain both HR Access Term Date and HR Immediate Term field. NOTE - Don't forget to publish your WF changes.

So, when HR or agent moves the issue to "TO DO", are they seeing the pop-up screen?

One other thing you may want to check to see if there are any automation rule configured for your project that somehow overrides your WF customization.

A few questions:
So it's not shown in the portal, but it's available for the agents. Do the hr folks have access to enter the info in the agent view? Who transition the ticket?

Yes. HR should have access and they transition the ticket from Waiting for approval to To Do.
It's still transitioning to To Do if custom field 1 & 2 is empty
