Hello,
What would be some simple workflow and settings to get request approval for manager?
I have created a request, added a form with fields:
Name - name of employee that is requesting vacation days
Department - department of employee
Date from/to - Dates between which he/she will be using vacation.
Manager - I added Users field here, from which requester will pick his manager from the list.
(it's connected to AD, so all users are available)
I would like to have approval step in workflow, manager gets notification, approves, and it's transitions to "Approved", then HR writes it up and they transition to "Done".
I tried playing with Workflows but probably I'm missing some check mark somewhere :)

Seems like you are on the right path. :-)
If you click on the workflow status while editing you can click: Include Approval step
Then click next and define approval source and number of approvers (approves will get notified by mail)
Click next and define what transitions should be used when approved or declined, then click add.

A little addition - You can under project settings -> customer notifications -> approval required define if the user needs to be logged in to approve or they can do it directly from the mail.

Thank you guys, you both are correct. That's the part I was missing.
I had everything set-up like on screenshot, but didn't marked that "Add approval step".
Now everything is working like how it should!
