Hi Jira Community,
Use case: I create a Jira issue and I add a form directly to the issue via the "Add Form" button located at the top of the issue. After I have added the form to the issue, I want someone else in my organisation to add some additional information to the issue via the form, and I want the form responses to be captured within the issue where I have added the form.
After I have added the form to the specific issue, I changed the form access to "External". However, I'm not able to find a unique URL for the form that I have added to the specific issue that I can share. Does anyone know how to do this?

Regarding your question, may I know why you need to find a unique URL for the form in order to share it? Why don't you just share the issue with the user so that they can view the form that you just added?

Thank you for your reply. The reason why I need a unique URL to the specific form attached to the specific issue rather than just sharing the issue itself is that the persons that I want to fill out the form are not Request Participants on the issue or Agents in the project.

In that case, I don't think it is possible in Jira Form.
Jira Form cannot stand on its own. To use the Jira Form, you must embed it within a Jira Issue or JSM's Request Type. The "External" option of the form means that the Form will be displayed in the JSM portal so that the Reporter or Request Participant will be able to fill up the Form from the portal.
I hope my answer clarifies how we use Jira Form in JSM.
