Do you need a confluence license to use JSM?

Welcome to the Community.
You only need a license if you want to use Confluence as a Knowledge Base, connected to JSM.
Only backoffice users (agents) that edit information in Confluence, need a Confluence license.
JSM Customers, who only access the KB via the portal, don't need a Confluence license.

if I understand you correct, you are using JSM and have registered users there where you do pay for already.
If you want now to introduce Knowledge-Base from within JSM (so Agents can update and Customers can Read), you do not need any additional licenses. Just if you want to use more Confluence features, licenses are needed.
Just try it out and create from JSM a KB and you will see.
