JSM form pushing to slack channel

I've created a form for a request type to have a user fill out and I want to show the information on the form in a slack channel.

I currently have the request type tickets populating to a slack channel, but since the form is not part of my ticket fields, the form data isn't showing on the slack channel, how do I complete this?

I also have the form creating a PDF, and was curious if that could be attached in a automated email when the ticket is created

I'm not sure if this can be done by Jira functions, but probably community leaders will tell you more.
However, I know for sure that it is possible to automatically send a custom message to the SLACK channel using the SLA Time and Report for Jira add-on.

It will be like an action when the SLA reaches a goal.
