Our customer is facing issue when creating request type from the help center to report bugs related to our mobile App. This is a request type we created to differentiate bugs reporting fro our different products (we also modify the icon)
Please note that other request type are working properly
Even after refreshing (as sugggested by atlassian) the issue remains.
This is urgent as our customer cannot report bugs properly
Do you use Forms for this request type? Or any apps? Please check the configuration. Maybe it causes some problems.

Yes, I use forms for my 2 bugs request types. The forms are very similar (only 1 checkbox section is different)
I removed the request type causing issues and bring it together with the request type that works.
Hope I won't face it again later if I need somehow to have different bug request type
